There are three ways to set up your new service:

1. Call 541-747-8072 and speak to our friendly staff to order recurring service or a one time delivery.
   Our office is open Monday through Friday, 9:00 a.m. to 5:30 p.m.
   
   
2. Print, fill out, and fax the appropriate following pdf forms to 541-747-8147:
